Dear Dermatologist: An Open Letter from My Frustrated Skin

“Dear Dermatologist, my name is Skin, and I’ve had it. Every day, my person jams these plastic things into my ears for hours. It’s hot, it’s sweaty, and frankly, I can’t breathe. I’ve tried sending signals—redness, itching, even a few angry pimples—but they just don’t get it. What can I do?”

A leading dermatologist responds: “Dear Skin, I hear your plea. Your person’s habit is creating what we call an occlusive environment. It’s like being wrapped in plastic on a hot day. This traps moisture and oil, which is why you’re feeling so irritated and are breaking out in a condition called acne mechanica.”

“You mentioned itching and redness. This could be a cry for help about an allergy. The silicone or metal in those earbuds might be triggering a reaction in you called contact dermatitis. This is different from the pimples, and it’s your immune system’s way of saying it doesn’t like the material it’s being forced to touch all day.”

“Your best defense is to convince your person to adopt a simple routine. They need to clean those earbuds daily with an alcohol wipe to remove the grime. They also need to give you a break—just five minutes every hour will let you dry off and get some fresh air. This will reduce both the breakouts and the irritation.”

“Finally, Skin, if you’ve been sending signals for a while and they’re still not getting better, your person needs to see one of my colleagues. Sometimes, the problem is a persistent infection or an allergy that needs a proper diagnosis and prescription treatment. Encourage your person to listen to you; you’re their first line of defense.”
